I hereby authorize the veterinarian of The Ridge Animal Hospital to perform the agreed-upon procedures. I understand the nature and purpose of the procedures, the risks involved, and possible complications that could arise. I understand there are no guarantees or assurances of the outcome of said procedures. I understand that while the anesthetic used in this hospital is one of the safest used in veterinary medicine, NO anesthesia is without medical risk. No guarantees can be made legally or ethically to me of any procedure performed. I release The Ridge Animal Hospital and the veterinarian from liability if something were to go wrong. Should an emergency arise calling for procedures in addition to, or different from those stated above, such procedure will be performed. I agree to pay in full for all services rendered, including those deemed necessary for medical and surgical complications or other unforeseen circumstances.

Pre-anesthetic blood work

The medical Staff at The Ridge Animal Hospital has made every effort to ensure your pet has a smooth and safe anesthesia and surgery/procedure. We use the safest anesthetic protocols available for your pet, but because no anesthesia is without some risk, our doctors recommend a pre-anesthetic blood screen for all patients undergoing anesthesia. We ask that all clients admitting a pet for general anesthesia approve or decline this procedure below. The pre-anesthetic blood screen is performed in our in-house lab. It enables the doctor to better assess anesthetic risk and/or prescribe intra-operative supportive measures to help ensure the safety of your pet.
